The Yor Community Wellbeing Hubs bring together a diverse partnership of health, statutory and voluntary sector organisations across the City of York.

Each partner contributes unique expertise and support, offering individuals a broader range of support tailored to their needs. This collaborative approach ensures that people can access the right help, in the right place, at the right time.


City of York Council

Collaboration is at the heart of everything City of York Council do. They are proud to work alongside a diverse network of partners with a shared commitment to making York a healthier, fairer, more sustainable, and more inclusive city for all. These partnerships span across public services, voluntary organisations, community groups, academic institutions, and private sector stakeholders - each playing a vital role in shaping the future of the city.

The council's partners help to deliver essential services, support vulnerable residents, promote economic growth, and enhance the cultural and environmental wellbeing of York. Whether it’s working with health and social care providers to improve outcomes for families, teaming up with local charities to tackle homelessness, or collaborating with universities to drive innovation and skills development, these relationships strengthen the ability of the council to respond to the needs of communities in York.

City of York Council and their partners support initiatives aligned with the Council Plan 2023 to 2027, which sets out a shared vision for a city where everyone feels valued and has access to opportunities. From climate action and clean air campaigns to inclusive transport and digital transformation, the council's partners help to turn strategy into action.

The council also work closely with regional and national bodies to ensure York’s voice is heard and that residents benefit from wider investment and policy support. These include partnerships through the York and North Yorkshire Combined Authority, NHS Integrated Care Systems, and national funding programmes such as the UK Shared Prosperity Fund.

Peasholme Charity

Peasholme Charity has a mission to end homelessness, and a vision that everyone has access to the right help at the right time to prevent the escalation of crisis situations.

Peasholme Charity works with disadvantaged and socially excluded people, in particular those who are homeless or at risk of losing their home.

They address social exclusion and poverty by providing support services for people who are in or heading towards crisis.

Their strategic aim is that people are empowered, realise their aspirations, and achieve their full potential.

Peasholme Charity work to:

  • reduce social exclusion, homelessness, and risk of becoming homeless
  • enable people to obtain and maintain a tenancy
  • enable people to engage in meaningful occupations, such as work, volunteering, education and training, and hobbies, or other recreational pursuits
  • increase individual motivation, self-efficacy and self-esteem
  • facilitate access to specialist services that meet identified needs and personal aspirations

My Money, My Life

The Peaseholme Charity My Money, My Life service is a financial capability advice and support service that offers a pathway from financial instability and crisis to a more secure and stable position.

My Money My Life offers:

  • a range of interventions from benefits advice to community drop-ins
  • long-term supported learning, aiming to provide individuals with the skills to prevent the recurrence of crisis situations
  • support with managing welfare benefits, for example Universal Credit and PIP applications, general benefits advice and benefit checks
  • support with managing utilities
  • cost of living support, with budgeting advice
  • crisis work, including issuing food bank vouchers and utility vouchers

Tees, Esk and Wear Valleys NHS Foundation Trust

Tees, Esk and Wear Valleys NHS Foundation Trust (TEWV) provide Mental Health and Learning Disability services to people living in York and Selby.

From education and prevention to crisis and specialist care, TEWV's talented and compassionate staff work in partnership with patients, communities and partners to help people to feel safe, understood, believed in and cared for.

TEWV are proud to play an important role in York’s Yor Community Wellbeing Hubs, and to have been involved in their development from the very beginning.

TEWV mental health practitioners are based within the Hubs to provide comfort, reassurance, advice and support for those needing help with their mental health or issues relating to their mental health.

The Hubs are collaboratively staffed by 4 partner agencies:

  • TEWV
  • York Mind
  • York Carers Centre
  • City of York Council

Each partner plays a vital role in offering comfort, reassurance, and support to those requiring help with their mental health. The knowledge and experience that TEWV staff bring ensures that people are helped to quickly access compassionate support that’s right for them. Whether this is providing initial guidance and advice, a referral for an assessment, or linking in with partners to help someone to obtain wider wellbeing support.

The Hub embraces a ‘no wrong door’ approach, meaning no matter what their needs, people are listened to and supported to access the help that’s right for them.

TEWV recognise that people’s mental health is largely affected by their environment, situation and relationships. Staff in the Hubs work closely with local community, voluntary and other public sector partners to make sure that people’s wider needs are addressed and that they are supported in a way that’s meaningful to them. By working together and addressing a range of needs at the same time the root cause of their mental health problems can be addressed, which can make a real and positive difference to people’s lives.

York Carers Centre

An unpaid carer is anyone who cares, unpaid, for a friend or family member who due to illness, disability, a mental health problem or an addiction cannot cope without their support.

Many carers do not recognise themselves as such - they are a family member or friend, simply doing their best for those they care about.

York Carers Centre is an independent charity and a network member of the national Carers Trust. York Carers Centre want unpaid carers, of all ages in York to be valued, recognised and supported to have a good quality of life. They work to challenge the inequalities that unpaid carers face, and ensure carers have access to support and services that reflect their needs.

York Carers Centre have 3 distinct services:

  • young carers, aged 5 to 18 years
  • young adult carers, aged 18 to 25 years
  • adult carers, aged 26 and over

York Carers Centre are proud to be a partner in the Yor Community Wellbeing Hubs, supporting those who care for someone with a mental health condition and who often have their own mental health needs. They support carers’ emotional and mental wellbeing through the Hubs and as part of their wider service.

York Carers Centre's core services provide:

  • advice and help navigating health social care systems
  • benefits advice
  • emotional support and counselling
  • specific services for carers of people with mental ill health and substance misuse issues
  • groups, activities, training and peer support
  • Carers Assessment of Need - for those caring for an adult
  • a discount card scheme
  • an emergency card scheme
  • bespoke support for young carers and young adult carers, including;
    • groups
    • events
    • one-to-one support
    • support in schools and colleges

York Carers Centre produce regular newsletters containing useful information, and encourage any carers or professionals to register for top tips, information and advice.

York Mental Health Partnership

The York Mental Health Partnership is a citywide collaboration transforming how adult mental health care is delivered. It brings together local NHS services, City of York Council, York St John University, voluntary sector organisations such as York Mind, and people with lived experience, to co-design accessible, person-centred mental health support rooted in the community.

At the heart of the partnership is a shared vision of “whole person, whole life, whole community,” inspired by international best practice - particularly the social care model from Trieste, Italy. The goal is to move away from institutional, hospital-based care and instead provide compassionate, holistic support through open, welcoming spaces embedded in neighbourhoods.

York Mind

York Mind are an independent charity delivering mental health services in York. They are part of the wider federation of local Minds and national Mind who work across England and Wales.

York Mind deliver a range of community mental health services for children, young people and adults. This includes:

  • advocacy
  • mentoring
  • counselling
  • training
  • coaching
  • peer support
  • social prescribing
  • specific support for LGBTQIA young people
  • a range of activities for young people from aged 11

York Mind are an active delivery partner in the Yor Community Wellbeing Hubs, and their staff deliver Peer Support and Social Prescribing.

Peer Support in Hubs

York Mind believe in the power of lived experience in championing and supporting a person's mental health recovery.

Their peer supporters all have their own lived experience of mental ill health. They can provide a non judgmental, support relationship, based on a shared understanding of knowing what it’s like to experience poor mental health, and focus on recovery.

You will be matched with a peer supporter who shares similar interests, hobbies and experiences.

Peer Supporters focus on 'what matters to you' most within your recovery and helps you to meet your goals and aspirations.

Social Prescribing in Hubs

Social Prescribing is designed to support people with a wide range of social, emotional or practical needs.

The York Mind Social Prescribing Team are focused on improving mental health and physical wellbeing, and follow a 'What Matters to Me' approach in supporting clients.

Social Prescribing is an all-age, whole population approach that works particularly well for people who:

  • have 1 or more long term conditions
  • need support with low level mental health issues
  • are lonely or isolated
  • have complex social needs which affect their wellbeing

Social Prescribing offers non-medical support - "We give patients time to explore what matters to them".

Social Prescribing aims to promote - "Clients' own ability to develop skills and strategies, and self-manage their own health and wellbeing".

Social Prescribers use an asset-based approach - "Shared decision making, to create a personalised patient care plan".

Social Prescribers link patients with local community support - "Voluntary, Community and Social Enterprise (VCSE) groups and statutory services".
